Welcome to Synergy Family Care. These terms and conditions govern your use of our services, including telehealth consultations, chronic care, acute care, wellness coaching, and any other health services we provide (collectively, the services). By using our services, you agree to these terms. If you do not agree, please do not use our services.
Definitions
Provider, we, us, our: Synergy Family Care and the clinicians and staff who deliver services.
You, patient, client: The individual receiving services or, where applicable, the legal guardian or authorized representative of that individual.
Telehealth: The delivery of health care services using electronic communication, information technologies, or other means between a patient and a provider.
Appointment: A scheduled telehealth consultation or in-person.
Eligibility and Registration
- You must be at least 18 years old to receive services. If you are under 18, a parent or legal guardian must provide consent.
- You must provide accurate, current personal and medical information when registering and keep it updated.
- Patients located in Florida are eligible for all services. Out-of-state clinicians providing telehealth to Florida patients will comply with Florida licensing and registration requirements.
Scope of Services
- We offer primary care and consultative services by licensed clinicians via telehealth. Services include acute care, chronic disease management, nutrition coaching, and home care aide coordination.
- The availability of specific treatments, tests, or prescriptions depends on clinical judgment, applicable law, and provider judgment.
- We are not an emergency service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Telehealth — Consent and Limitations
- By booking a telehealth appointment you consent to receiving care via telehealth. You acknowledge and accept the limits and risks of remote care, including technical failures, limited physical examination, and potential privacy risks.
- Telehealth may not be appropriate for all conditions. A provider may recommend an in-person visit when clinically necessary.
- You must be in a private, secure location for telehealth visits. You agree not to record sessions unless you have written consent from the provider.
Appointments, Cancellations, Rescheduling and Refunds
- Appointments must be booked online or by phone. Payment is due at the time of booking unless otherwise agreed.
- To receive a full refund, you must cancel or reschedule your appointment at least two (2) hours before the scheduled appointment time. If you cancel or reschedule less than two (2) hours before the scheduled appointment time, the appointment is non-refundable.
- No-shows and late arrivals are non-refundable. Providers will make reasonable efforts to complete the session within the scheduled time, but sessions may be shortened if the patient is late.
- We may, at our discretion, offer exceptions to the refund policy for documented emergencies or extraordinary circumstances. Any exceptions are made only by authorized staff and documented in the patient record.
- If Synergy Family Care cancels or reschedules an appointment, you will be offered the option of rescheduling or receiving a refund for any prepaid fees.
Fees, Payment, and Billing
- Our services operate primarily on a cash-pay basis. Payment methods accepted are listed at booking. You are responsible for paying all fees at the time services are scheduled or rendered.
- Fees for telehealth are posted on our website and subject to change. We will notify you of material fee changes when possible.
- We do not bill insurance unless agreed in writing. If you choose to submit charges to your insurer, you are responsible for accurate submission and any differences.
- Returned payments, card chargebacks, or failed transactions may result in additional fees and suspension of services until resolved.
Medical Records and Privacy
- We maintain medical records in accordance with applicable law. Your health information is protected under federal and state privacy laws.
- Telehealth sessions use secure platforms. Despite safeguards, you acknowledge that no system is perfectly secure. For information about how we protect data and your rights, review our Privacy Policy.
Prescriptions and Controlled Substances
- Prescriptions will be provided only when clinically appropriate and in accordance with applicable laws and professional standards.
- Some medications, especially controlled substances, are restricted or may not be prescribed via telehealth. Providers must follow Florida law, federal rules, and professional guidelines when prescribing. You agree that the provider may decline to prescribe certain medications based on clinical judgment or legal restrictions.
Patient Responsibilities
- Provide complete and accurate medical histories, current medications, allergies, and relevant health information.
- Follow provider instructions, complete recommended tests, and ask questions when unclear.
- Ensure the telehealth environment is private and secure. Arrange any necessary technology and a stable internet connection.
Confidentiality and Data Use
- We use your health information to provide services, process payments, and improve care. We will not sell your protected health information.
- In limited situations we must disclose information by law, such as mandatory reporting of certain infectious diseases, abuse, or court orders.
Risks, Disclaimers, and No Guarantees
- Medical care carries risks. We do not guarantee results, cures, or specific outcomes. Diagnoses and treatment plans are based on available information and clinical judgment.
- Telehealth involves limitations compared with in-person care. You accept that remote evaluation may not capture all clinical findings, and providers may recommend face-to-face visits when necessary.
Limitation of Liability
To the fullest extent permitted by law, Synergy Family Care, its owners, providers, employees, and contractors will not be liable for indirect, incidental, special, punitive, or consequential damages, including lost income or emotional distress, arising from your use of the services. Our total liability for direct damages will not exceed the amount paid by you for the affected service in the three months preceding the claim.
Indemnification
You agree to indemnify, defend, and hold harmless Synergy Family Care and its personnel from claims, damages, liabilities, costs, and expenses arising from your breach of these terms or your misuse of the services.
Dispute Resolution and Governing Law
These terms are governed by the laws of the State of Florida, without regard to choice-of-law rules.
Any dispute arising under these terms will be resolved first through informal negotiation. If unresolved, disputes will be resolved through binding arbitration in Florida, except where prohibited by law. Small claims actions may be brought in the appropriate court.
Termination and Suspension
We may suspend or terminate your access to services for nonpayment, violation of these terms, or for any reason when we reasonably believe continued access poses a risk. Termination does not relieve you of outstanding fees.
Changes to Terms
We may modify these terms at any time. The updated Terms will be posted on our website with the effective date. Continued use of services after changes means you accept the revised terms.
Third-Party Services and Links
We may use third-party vendors for telehealth platforms, payment processing, or laboratory services. Use of third-party services is subject to their terms. We are not responsible for third-party practices.

Additional Provisions
Severability
If any provision is found invalid, the remaining provisions remain in effect.
Waiver
Our failure to enforce a right is not a waiver of that right.
Entire Agreement
These terms and any documents incorporated by reference form the entire agreement between you and Synergy Family Care.
