Thank you for visiting Synergy Family Care. This Disclaimer explains important limits to the information and services available through our website, patient portal, telehealth visits, and in-home care. Read it carefully. Using our site or services means you accept these limits. If anything below is unclear, contact us.
This document explains what our content and services are for, and what they are not. It clarifies legal limits, safety boundaries, and how you should use the information you read or hear from our clinicians. It does not replace professional medical advice or your clinician’s instructions.
The information on our website, blog posts, educational materials, and general guidance from clinicians is for informational and educational purposes only. It is not intended to replace professional medical advice, diagnosis, or treatment.
Do not ignore or delay seeking medical advice because of something you read or hear from us. If you have a medical emergency, call 911 or go to the nearest emergency department.
We aim to keep content accurate and current. Medical knowledge changes. We cannot guarantee that every article, resource, or answer is complete, up to date, or free of error. You should not rely solely on the site for clinical decisions. Always check with a clinician before starting or stopping any treatment.
Accessing our website, reading articles, or using general information does not create a doctor–patient relationship. A formal clinician–patient relationship exists only when a clinician has agreed to provide care and you have completed the intake and consent processes required by Synergy Family Care.
Telehealth is a convenient way to receive care, but it has limits. Remote consultations may not reveal all physical signs a clinician could see in person. Technical failures, network interruptions, or poor image/sound quality can reduce the accuracy of assessments.
If your clinician determines in-person evaluation, testing, or emergency care is needed, you must follow that recommendation. Telehealth services are not appropriate for all conditions. Synergy Family Care reserves the right to recommend or require an in-person visit when clinically necessary.
When clinically appropriate, providers may prescribe medication. However, certain medications, including many controlled substances, may be restricted or refused for telehealth prescribing in compliance with federal and Florida law.
You agree that clinicians may decline to prescribe particular medications based on clinical judgment, safety concerns, or legal restrictions. You must follow prescription instructions precisely and notify your clinician of allergies and other medications you take.
Synergy Family Care does not provide emergency services. If you are experiencing chest pain, difficulty breathing, severe bleeding, loss of consciousness, or any life-threatening symptom, call 911 or go to the nearest emergency room immediately. Do not use telehealth for emergencies.
Our site may link to third-party websites, tools, or resources. These links are provided for convenience and do not imply endorsement. We are not responsible for third-party content, accuracy, or privacy practices. Use third-party resources at your own risk.
Patient stories, reviews, and testimonials reflect individual experiences and may not predict results for everyone. Testimonials do not constitute medical advice or guarantees of outcomes. Individual results vary.
We strive for uninterrupted access, but we do not warrant that the website, portal, or telehealth platforms will be error-free, secure, or always available. Downtime, software bugs, or cyber incidents can occur. You agree that use of the services is at your own risk.
Fees, payment terms, and refund policies are described in our terms and Conditions and at point of sale. Use of our site or scheduling tools does not guarantee a specific fee, insurance coverage, or reimbursement. If you submit payment information, you confirm it is accurate and authorized.
We take privacy seriously. Our Privacy Policy and HIPAA Notice describe how we collect, use, and protect your information. However, no digital system is perfectly secure. By using our services, you acknowledge inherent risks in electronic communication and agree to the data practices described in our Privacy Policy.
To the maximum extent permitted by law, Synergy Family Care and its staff, clinicians, contractors, and affiliates will not be liable for indirect, incidental, consequential, punitive, or special damages arising from your use of our website, content, or services.
Our aggregate liability for direct damages will not exceed the amount you paid for the affected services in the three months prior to the claim. Some jurisdictions do not allow limitations of certain damages; if any limitation is invalid, only that portion will not apply.
You agree to indemnify and hold harmless Synergy Family Care and its representatives from claims, damages, losses, expenses, and liabilities arising from your use of the site, violation of this Disclaimer, or breach of applicable law.
All site content, including text, logos, images, and downloadable materials, is owned or licensed by Synergy Family Care unless otherwise stated. You may not reproduce, distribute, or create derivative works without written permission.
This Disclaimer and your use of our services are governed by Florida law. Any disputes will follow the dispute resolution terms in our terms and Conditions. If any provision is unlawful or unenforceable, it will be severed, and the remainder will stay in effect.
We may update this Disclaimer periodically. We will post the effective date at the top and notify users when material changes occur. Continued use after updates means you accept the revised terms.
